  12. Darparu pecyn cymorth i ddefnyddwyr gwasanaeth sy’n symud ymlaen o'r Cynllun, am gyfnod dros dro lle bo hynny'n briodol, sicrhau bod yr holl waith papur perthansol wedi’i gwblhau a’i anfon at yr asiantaethau perthnasol.
  13. Cynorthwyo gyda rheoli tai a gosodiadau fel sy'n ofynnol gan Reolwr y Cynllun yn unol â pholisïau a gweithdrefnau TGC.
  14. Ymateb a delio ag unrhyw ddigwyddiadau yn briodol, cysylltu a chyd-gysylltu â staff Tai Gogledd Cymru, y gwasanaethau brys, asiantaethau gwirfoddol a statudol, ayb er mwyn cynnal diogelwch y staff, defnyddwyr gwasanaeth a’r llety ar bob adeg.
  15. Darparu gwasanaeth y tu allan i oriau arferol, ar sail rota, yn ôl y gofyn.
  16. Cysylltu'n effeithiol gydag Adran Rheoli Asedau TGC a chontractwyr ynghylch gwaith cynnal a chadw ac atgyweirio eiddo TGC.
  17. Cyfrifoldeb am fynychu cyfarfodydd partneriaeth allanol, gan roi diweddariadau ar ran y cynllun i asiantaethau partner sy’n mynychu cyfarfodydd.
  18. Cefnogi Rheolwr y Cynllun trwy sicrhau bod safonau iechyd a dioglewch yn cael eu cynnal yn y cynllun e.e. cwblhau gwiriadau ystafell
  19. Sicrhau bod tasgau cadw tŷ a glanhau yn cael eu cynnal yn rheolaidd a, lle bo angen, darparu cymorth ac arweiniad ymarferol i ddefnyddwyr gwasanaeth, gan sicrhau bod safonau da o ran glendid a hylendid yn cael eu cynnal. Yn ogystal - o fewn yr Hostel a tai rhannu-sirchau fod y glanhau a thasgau cadw tŷ yn cael eu cwblhau yn aml pan sy’n ofynnol.
  20. Paratoi ystafelloedd ar gyfer eu hail-osod, yn ôl yr angen.
  21. Cefnogi Rheolwr y Cynllun drwy gysylltu’n effeithiol gydag adrannau a’r Gwasanaeth perthnasol o fewn TGC (e.e. Anghenion Tai Cyffredinol, Rheoli Asedau, Cyllid ayb) ynglŷn â diogelwch yr adeilad(au), cynnal a chadw, rheoli ôl-ddyledion, rhenti a rheoli arian parod symud-ymlaen, ayb.
  22. Trefnu a rheoli amrywiaeth o weithgareddau ymgysylltu a chynnwys er mwyn hyrwyddo cynhwysiant cymdeithasol ymysg defnyddwyr gwasanaeth.
  23. Gweithio'n annibynnol pan fo angen, a hefyd fel aelod o dîm sy'n seiliedig ar ardal.

JOB PURPOSE

 

  • To provide an effective and efficient housing management and support service, in line with the service specification and requirements of funders, stakeholders and the direction of the Manager.
  • To support service users with a view to ensuring their progress towards the outcomes, defined within a clearly written Support Plan.
  • To contribute to the safe and effective day to day management of the Scheme.

 

ACCOUNTABILITIES

Statement of the main areas of responsibility

  1. Be responsible for drawing up, regularly reviewing and reporting on Risk Assessments and Support Plans, working towards outcomes within the requirements of the Supporting People Programme – fully involving service users.
  2. Be responsible for drawing up and regularly reviewing individual risk assessments, fully involving service users in their review where appropriate.
  3. Maintain accurate service user records and files, preparing electronic and paper reports, as required.
  4. Be responsible for completing all move in paperwork with new residents in timely manner, ensuring a comprehensive Induction to the Scheme is provided to all new service users (including completing applications for housing and related benefits)
  5. Be responsible for managing referrals to the scheme, contacting and interviewing applicants/agencies, arranging panel meetings, updating staff in scheme of new referrals.
  6. Be responsible for logging service charges payments/donations on internal systems and sending weekly updates to staff and Rents department.
  7. Be responsible for issuing verbal and/or written warnings as directed by Scheme Manager; ensuring all paperwork is kept up to date.
  8. Liaise and network effectively with voluntary and statutory agencies, ensuring the provision of consistent good quality support and advice to service users.
  9. Undertake training to maintain and improve the skills needed to undertake this role.
  10. Provide a range of housing related support to service users with a view to enabling them to manage their individual tenancies with less support, and possibly unaided in the longer term, in a timescale agreed with the Manager.
  11. Assist service users with plans to move on to more permanent accommodation – where relevant - by liaising closely with relevant NWH staff, local authority Housing Departments and other providers as necessary.
  12. Provide a package of support to service users moving on from the Scheme, for a temporary period where appropriate, ensuring all relevant paperwork is completed and sent to the relevant agencies.
  13. Assist with housing management and lettings as required by the Manager in accordance with NWH’s policies and procedures.
  14. Respond to and deal with any incidents appropriately, contacting and liaising with NWH staff, emergency services, voluntary and statutory agencies, etc. to maintain the safety of the staff, service users and accommodation at all times. Provide out of hours, on call cover on a rota basis, as required.
  15. Liaise effectively with NWH’s Asset Management Department and contractors regarding the maintenance and repair of NWH property.
  16.     Responsibility for attending external partnership meetings, providing updates on behalf        

    of the scheme to partner agencies in attendance at the meetings.

 

  1.    To support Scheme Manager by ensuring H&S standards are maintained in the   

   scheme e.g. completing room checks

 

  1. Ensure service users maintain the property and where necessary provide practical support and guidance to service users on how to maintain good standards of cleanliness & hygiene within the property. In addition – within Hostel accommodation and Shared Housing - ensure housekeeping and cleaning tasks are regularly carried out as required. Prepare rooms for re-letting, where shared housing or Hostel accommodation is provided. Support the Manager in liaising effectively with relevant departments and Service’s within NWH (e.g., General Needs Housing, Asset Management, Finance etc.) regarding the safety of the building(s), maintenance, arrears control, rents and cash control  move-on, etc. Organise and manage a range of engagement and involvement activities to promote social inclusion amongst service users. Work independently when needed and also as a member of an area-based team. Safeguard the physical and emotional health and wellbeing of service users.

QUALIFICATIONS/EXPERIENCE/KNOWLEDGE/SKILLS

Tools needed to do the job

Essential

  1. A minimum of 3 GCSEs or equivalent at Grade C, or above, including mathematics and English.
  2. Good written and verbal communication skills.
  3. Experience of working in a client centred environment (e.g. supporting individuals in reporting maintenance issues, making healthcare appointments, pursuing hobbies, securing training and employment, etc.).
  4. Effective partnership working skills.
  5. An awareness of the support needs of vulnerable people
  6. A non-judgemental approach to supporting vulnerable people
  7. The ability to work alone with confidence and good judgement
  8. Ability to provide out of hours, on call cover on a rota basis
  9. Experience or understanding of the need to work in a confidential manner
  10. Able to understand and stay within professional boundaries.
  11. The ability to use Microsoft Office products (Word, Excel and Outlook).
  12. Good administrative skills for record keeping and assisting clients with claims for benefits
  13. Experience of working in a customer focused organisation.
  14. A full driving licence and the availability of a vehicle for work purposes.
  1. Is comfortable with everyday conversations/discussions specifically in relation to your job.  Be confident to converse in Welsh when required. (This requirement is set at Level 3 on self - assessment form)

Desirable

  1. Knowledge of mental health issues / support needs connected to drug and alcohol misuse / homelessness
  2. Knowledge of local specialist agencies e.g. drug and alcohol, mental health services etc
  3. A successful track record of working in a housing management or community based support environment for individuals suffering or recovering from mental illness
  4. A Diploma in Welfare Studies, HNC in Housing Studies or Mental Health qualification
  5. An NVQ / CQF Level 2 in Health and Social Care.
