You’ll be joining us on our exciting journey to become market leaders in New Models of Care by 2021. This means we’re not providing your traditional supported housing services but rather trailblazing the way in delivering brilliant long term outcomes for vulnerable people through integrated health, housing and care.

 
We have two brilliant opportunities for Occupational Therapists to join our services across County Durham, which provides support to young people (aged 16 to 25 years) and adults over 18 years with mental health issues and complex needs.  Our service enables people to move on from 24 hour supported living, to semi-independent shared living, to living independently in the community – how amazing is that!  You’ll be working alongside a Mental Health Nurse and team of Support Workers, ensuring that high quality individualised care is delivered which ensures safety, recovery, wellbeing and maximises independence.
 
You’ll be supporting our customers to identify the things they want and aspire to do in their lives.  This will include the assessment and management of a range of complex occupational therapy needs, diverse presentations and a range of mental health conditions, cognitive and/or learning disabilities.  You’ll identify where skills development is required and support customers to achieve their goals through everyday activities.  You’ll work closely with our Support Workers to deliver personalised interventions, develop recovery and wellbeing focused plans, treatment planning as well as regular reviews.

On a day to day basis, you’ll report to Kara our Senior Client Services Manager with your clinical supervision provided by our Clinical Practice team.  The team love to catch up each week to discuss new interventions that have really helped our customers.  They also make sure there’s plenty of cake on the go and time in the agenda to discuss the latest episode of Game of Thrones. So how’s it going to end?

What’s even more brilliant, you’ll be able to really make your mark (there’s no red tape or mounds of paperwork here!).  You’ll be shaping our services, clinical design and delivering specialist training which unleashes the potential of our colleagues.

A bit about you
 
You’ll have experience of working with adolescent and adult mental health, offering occupational therapy based interventions.  You’ll hold relevant professional qualifications including HCPC Occupational Therapist.   You’ll really impress us with your ability to understand where our customers are in their lives, using evidence based practice, care delivery and current occupational therapy based interventions.

As the roles cover our services across County Durham, you'll need to have a full current driving licence and access to a car for work purposes with business insurance cover.  We'll pay you for your mileage.
 
An Enhanced Check under the Disclosure and Barring Service is required for this role and will be paid for by us.
