Specialist Mental Health Support Coordinator x4
We are looking for a number of Specialist Mental Health Support Coordinators to join our Mental Health Team providing a Recovery focussed model of Floating Support.
You will be supporting adults with both complex and enduring mental health problems in their own homes. The support you provide will be varied being both practical and emotional and be delivered within a recovery focussed model of support. It will include completing needs and risk assessments with the service users, giving advice on income benefits, accessing employment or training needs and liaising with other health professionals to empower the service users to maintain their independence and lead fulfilling lives within their community.
Supporting a caseload of service users, you will be office based with the team while managing your time to visit service users in their own homes and in the community. You should have experience of understanding the support needs of people with Mental Health problems, gained either in the field, classroom or in a life capacity and be committed to the continued development and excellent work of this service.
The ideal candidate will have the following experience:
- Relevant NVQ level 2 or relevant qualification
- Substantial experience of delivery support services to people with enduring mental health problems
- Understanding of issues involved in supporting people in the community with mental health problems
- Ability to act independently & where & when to seek the support of other agencies
- A broad understanding of the delivery of recovery based mental health services
You must be able to work shifts between 8am-10pm Monday-Sunday - shifts are based over 7 days.
