We are seeking an experienced care and support worker to join our team in Baldock. If you are looking to join a leading Housing Trust that will support your development then we would be interested in receiving your application.

This scheme supports 23 customers over 5 services. Each customer has unique learning disabilities and individual care requirements. The focus is on them maintaining independent living and achieving social inclusion.

You will be required to work flexible over a seven day rota including bank holidays. The rota is 37.5 hours a week and includes earlies, 7.30 am - 3.00pm, lates 2.30pm - 10.00pm and very occasionally sleep ins

Key responsibilities will include , but are not limited to:

-Work with customers to develop, review and update of care and support plans and risk assessments in line with policy.
- Deliver care and support in line with individual care and support plans and risk assessments.
-Encourage and support customers to establish and maintain positive relationships
-Provide support and practical assistance to customers around benefits, budgeting, paying bills and accessing appropriate services, including some housing management.
-Promote self advocacy and act as the customers` advocate where appropriate.
-Be aware of customers` wellbeing, safety and state of health, reporting any concerns.
-Work collaboratively with colleagues in other teams to improve service effectiveness and deliver a positive customer experience.
-Build and maintain effective and productive working relationships with colleagues in the business and a broad range of external stakeholders
-Encourage and enable customers to be involved in shaping the service and its offering and their own care and support
-Achieve high customer satisfaction levels using feedback and customer insight
-Proactively deal with enquiries from family members, social workers and health professionals.

As a senior support worker you will need experience to be successful in your application which should be clearly demonstrated in your CV.

To be considered for this role your CV will need to reflect: -

1)Experience of working as carer
2)Knowledge of care plans and how they should be maintained
3)Understanding the handling of medication and safeguarding
4)IT literacy

If you have experience of providing supported living for customers with learning disabilities this will be an advantage.
