Covering our services across County Durham

Brilliant outcomes through integrated health, housing and care. That’s when it hits home.

This is your chance to be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily support. You’ll be joining us on our exciting journey to become market leaders in New Models of Care by 2021. We’re trailblazing the way in delivering brilliant long term outcomes for vulnerable people through integrating health, housing and care.

We have a brilliant opportunity for you to join our services across County Durham, which provides support to young people (aged 16 to 25 years) and adults over 18 years with mental health issues and complex needs. Our service enables people to move on from 24 hour supported living, to semi-independent shared living, to living independently in the community – how amazing is that!

As a Registered Mental Health Nurse, you’ll be developing our Support Workers to ensure that high quality individualised care is delivered which ensures safety, recovery, wellbeing and maximises customers independence.

As an experienced clinician, you’ll lead practice development, developing and delivering specialist training packages to our colleagues, embedding core competencies as well as oversight of clinical supervision.  You’ll also be the point of contact for individual support and coaching of the Managers in our services.  You’ll be really enhancing colleagues’ knowledge and skills, promoting regular, honest and supportive feedback.

What’s even more brilliant, you’ll be able to really make your mark (there’s no red tape or mounds of paperwork here!).  You’ll be shaping our services, developing essential standards and quality outcomes.  With customer care at the forefront of everything we do, you will inspire others and advocate a philosophy of care that is person centred, maintains dignity, independence and ensures it is right for that person at that time.

You’ll enable us to maintain high standards of support and intervention to contribute to achieving the Care Quality Commission Essential Standards.

On a day to day basis, you’ll report to Kara our Senior Client Services Manager with your clinical supervision provided by our Clinical Practice team.  The team love to catch up each week to discuss new interventions that have really helped our customers.  They also make sure there’s plenty of cake on the go and time in the agenda to discuss the latest episode of Game of Thrones. So how’s it going to end?

A bit about you

You’ll have an eye for detail, being able to analyse complex data and make expert judgements. You’ll really impress us with your ability to unleash the potential of colleagues through offering clinical advice, coaching, mentoring and training.  You’ll also bring a creative spark and ability to understand where our customers are in their lives, using evidence based practice, care delivery and current occupational therapy based interventions.

An Enhanced Check under the Disclosure and Barring Service is required for this role and will be paid for by us

Working flexibly

It’s our unwritten rule that you attend the important appointments in life, be it your child’s school play, your partner’s graduation, or to get your new fridge delivered. You can elect to fit these around your working day or use your flexi to take the time off. Our focus is on getting the job done, not your working hours!  And we really mean this!

The job for you?

Great, all you need to do now is upload a covering letter and your CV.  Easy as that!

Interviews are planned for 29th April so keep this date free in your diary.
