Brilliant outcomes through integrated health, housing and care.  That’s when it hits home.

You have a chance to be part of something brilliant at Home Group, where our exciting journey to become market leaders in New Models of Care is really coming to life in the Yorkshire region. We have plans to deliver seven new Community Wellbeing (Extra Care) Schemes across Yorkshire in the next two years providing integrated health, housing and care for older people.

Join us and lead the implementation of these fantastic new services.  This is a brilliant opportunity for you to really make your mark, leading on the operational implementation of these innovative projects and getting outstanding results for our customers and organisation. You’ll be project managing successful mobilisation, co-ordinating activity across internal and external partners, naturally collaborating to ensure commercial success and excellent outcomes for our customers. You will be creating standard implementation processes using your creative and collaborative approach and directly inputting into design, staff recruitment and operational procedures, working alongside our local operational and national New Models of Care teams.

You’ll be working for Sarah Clark, our Head of Service Delivery for Yorkshire and alongside 3 fantastic Operations Managers. So if you want to do awesome things in a brilliant team and work for an organisation that is shaping the future in new models of care…join us at Home Group.

A bit about you

You’ll have previous experience of successfully developing and implementing new services within a housing, care and support environment using your understanding of rent and service charge setting.  With fantastic analytical and problem solving skills you’ll be a skilled presenter and adept at determining business priorities, making recommendations and decisions for change.  You’ll really impress us with your ability to build relationships, influence senior stakeholders, manage projects and produce reports/complex briefing documents.

 A bit about us

We’re Home Group one of the UK’s biggest providers of high quality housing and care and support services.  We have a wealth of expertise and deliver a wide range of specialist and support services to people with complex health and social care needs.  We recognise the diverse needs communities have and provide a team of specialists and clinicians within our business, supporting the NHS’s commitment to improving health and wellbeing through innovative new models of care.

And also, did we mention that we rank no. 7 in the UK’s Best Super Large Places to Work and 17^th in Stonewall’s Top 100 employers.

Working flexibly

It’s our unwritten rule that you attend the important appointments in life, be it your child’s school play, your partner’s graduation, or to get your new fridge delivered. You can elect to fit these around your working day or use your flexi to take the time off. Our focus is on getting the job done, not your working hours!  And we really mean this!

What’s on offer?

Through our brilliant learning pathways, we’ll support you with your own continual professional development (will even pay for your relevant professional membership fees too) as well maintaining your professional registration through regular clinical supervision and reflective practice.
