We’re looking for a customer-focused individual with energy, experience and a strong sense of purpose to join us as a Community Link Worker.

Catalyst is leading by example as the first social prescribing initiative in the Borough of Ealing following a successful pilot last year. Social prescribing is when health professionals refer patients to support in the community in order to improve their health and wellbeing; Catalyst plays an important part by working with communities to tackle important local issues such as poor health and social isolation.

As a Community Link Worker you will lead the delivery of our ‘Social Prescribing Southall’ project. You will provide personalised support; meeting people on a one-to-one basis and helping them to improve their physical health and mental well-being by directing them to community providers.

You will develop strong relationships with local GP surgeries and the Primary Care Network to ensure the success of the project and establish partnerships with local voluntary and community groups supporting them to receive social prescribing referrals.

You will manage a small team of volunteers who will work with you to champion social prescribing and administer the project.

You will be able to communicate effectively, both verbally and in writing, with individuals, community groups and strategic partners.

You will follow Catalyst’s information governance policies and procedures to ensure GDPR compliance whilst capturing key information about participating individuals to track the impact of social prescribing on their health and wellbeing.
