Brilliant outcomes through integrated health, housing and care. That’s when it hits home.

Be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily care and support.   We’re delivering brilliant long-term outcomes for people who have complex support needs through integrating health, housing and care.

We have a fantastic opportunity for you to join our brand-new Transforming Care service in St Albans, delivering specialist Positive Behavioural Support to our customers.   Working alongside our Registered Manager, PBS Team Leaders and Support Workers, you’ll be supporting eight of our wonderful customers, who are all young adults (aged 19 to 25) living within their own flats within our communal house.  Our customers have varying levels of learning disabilities with a huge amount of potential to develop their skills to be as enabled and independent as possible.  For some customers, their needs are more complex with both learning and physical challenges and will need support and reassurance to help them make that exciting first step to moving out of their family home to living independently on their own.

We are currently in the process of renovating their home ‘Nicholas House’ which is beautiful old building with a communal lounge for social, therapeutic and group activities, sparkly new office for the team and a big garden (which has lots of space to grow plants or veg if our customers fancy that!).  It will be all ready for our customers to move in by April 2021.

As our PBS specialist you’ll be leading on functional behavioural assessments, plans and interventions, working collaboratively with our customers, their families, clinical and operational colleagues.  You’ll support the transition of customers into their home, some who may display behaviours that challenge, planning for their move and identifying risks. You’ll use your experience and brilliant skills to ensure all practice is evidence based and person led. You’ll provide PBS advice and coaching to your colleagues on the day to day support required, ensuring safety, promoting recovery, wellbeing and maximising independence.

You’ll report to Sally our new Registered Manager who is excited to be joining us at the end of this month and you’ll also be supported by Chelsea (and for your clinical supervision too) who is our Regional Clinician – PBS.

Where you’ll work

You’ll work with our customers as they transition and move into their own home in St Albans, as well as supporting them in the community. Their home is on a lovely quiet road, with lots of shops, great bus routes and a train station nearby at St Albans – perfect for the commute or a day out!

A bit about you

You’ll be an experienced PBS specialist, with a relevant professional qualification such as Level 5 Registered Manager or Degree in Learning Disability, Mental Health, autism or other health and social care.  You’ll also have a post-graduate qualification in PBS/Applied Behaviour Analysis.   You’ll be experienced in working with a wide variety of customers with complex needs including learning disabilities, mental health, autistic spectrum condition, and behaviours that concern.  You’ll have a creative spark, be solutions focused and brilliant at supporting and unleashing the potential of our customers and operational teams to work with them through effective delivery of PBS.

You must be able to drive and have access to a car for work as you'll be required to travel to support customers transition.

An Enhanced Disclosure Check will also be required for this role and the costs of this will be met by us.
