Brilliant outcomes through integrated health, housing and care. That?s when it hits home.
Be part of something brilliant at Home Group, where clinical interventions work hand in hand with daily care and support. We?re delivering brilliant long-term outcomes for people who have complex support needs through integrating health, housing and care.
We?re looking for a brilliant Occupational Therapist to join our brand-new trauma informed women?s service in Leeds. You?ll lead on the assessment and management of a range of complex occupational therapy needs, providing clinical advice to our colleagues on the day to day support required. This is an exciting opportunity to help us shape this brand-new service, working with our customers to recognise their strengths, identify the things they want and aspire to do in their lives and achieve their full potential. You?ll be enabling our customers to live safe, happy and fulfilling lives within the local community, who otherwise could have remained in locked mental health wards or privately-run facilities. How amazing is that?
Our customers all have complex support needs with a history of trauma, including learning disabilities, Autism, a diagnosis of mental health conditions, personality disorders, Psychosis and PTSD. Some of our customers may also have a forensic background and support needs related to their behaviour. Some may have experienced difficult and traumatic childhoods, chaotic adult relationships or may have children who they are estranged from.
We have single occupancy properties in one area of Leeds, with each property being bespoke to the customer moving in, which includes their own garden area. The first property will be ready for our first customer to move in by end of July 2021.
You?ll work closely with our support team to deliver personalised interventions, behaviour support plans and identify where skills development is required so our customers can achieve their goals through everyday activities. Together, you?ll be delivering safe care, that improves our customers quality of life and maximises their independence.
You?ll report to Dipty our Clinical Registered Manager and be supported by our wider New Models of Care clinical team who come from all backgrounds including nurses, behavioural experts and our brilliant group of OT?s.
