As Nurse Consultant in this brilliant brand-new role at Home Group, you’ll help us shape the strategic direction of mental health nursing practice across our Transforming Care services, creating frameworks and extending the boundaries of excellence in our nursing care.  As senior leader, you’ll deliver and oversee specialist nursing assessments, formulations and interventions, managing clinical risk to aid the transition of care and support for customers with high risk needs. Our customers have a wide range of complex and interconnected needs including learning disabilities, autism, a diagnosis of mental health conditions, personality disorders, communication differences, alongside ongoing physical health conditions.  They may also have a forensic background and support needs related to their behaviour.

You’ll also oversee medication management including de-prescribing and leading on STOMP as well as embedding new interventions and practices aligned to our Living Independently Feeling Enabled (LIFE) support practice model.

You’ll report to Nic, our Director of Clinical Practice, who you’ll find outside of work getting out in the country for walks with her daughter and small but powerful Cavapoo. She has amazing vision for developing the future of new models of care and would love you to join her and the team on this journey.
