About the Service/Department

The Hounslow Hospital Discharge service aims to prevent people who are homeless on admission to hospital from being homeless on discharge. It supports people to sustain secure accommodation and improve their health with a view to preventing further admissions. Service users may present different needs such as mental health, substance misuse or physical health for example.

About the role

Ensure effective service delivery, staff supervision and case load management. To be responsible for case work for people with a wide range of mental Health issues to promote their independence and wellbeing and support them to achieve their goals

About you

• An understanding of the support needs of older adults
• Excellent Knowledge of issues related to signs, symptoms and treatments of mental illness. Ability to assess service users’ declining mental health and take appropriate action
• A detailed understanding of key-working, personalised support, and coproduction
• An awareness of safeguarding procedures and risk assessment principles
• Commitment to working within an Equality & Diversity Framework
• Knowledge and understanding of safeguarding issues and ability to address them appropriately
