Job Details

Gloria House is a hospital discharge service that is staffed Mon-Sun from 8am – 9pm. The customers are people who are medically fit to be discharged but who would otherwise be homeless. The client group may include people who have support needs around mental health or have a history of offending, substance use and/or fleeing domestic abuse or gang violence. 

As a Resettlement Officer will work with the rest of the team in supporting our customers to achieve their goals. The Service supports people who people who are medically fit to be discharged from hospital but who are homeless. The people may have support needs around mental health or have a history of offending, substance use and/or fleeing domestic abuse or gang violence. The people who use the service will stay for a maximum of 6 weeks so you will be working intensively for a short period to support people to get back on their feet and move into permanent accommodation. There may be some customers with complex issues. You will be allocated a caseload to keywork, you will also be expected to complete admin tasks such as updating the internal systems with notes, completing risk/need assessments and supporting customers to attend meetings.

You will need to communicate with external services and internal teams, follow up on actions, complete incident reports, raise safeguarding and support customers with ADLs and tenancy management.

You will also be responsible for monitoring H&S and repairs and with the tasks related to assessing new referrals to the service.

You will need demonstrate the values that you will need to work with this sometimes challenging client group and to have the confidence once inducted to lone work.
